Great program. Based on all the questions, this is what I've found out (I've had this whole thing for about 1/2 hour, so dont flame me if its wrong).
Always on top: Right click and go to Properties. Within that window, goto the settings area. Change "Z Order" from desktop to always on top.
Different buttons/sizes: I think the button sizes are based on the graphic you use for the button. Most themes should have graphics for the buttons, if not find another one :-).

Always on top: Right click the bar, goto properties. Under settings, fine Z-Order and change it to always on top. I hope all you guys that trashed this plugin for not being always on top feel really stupid now.
Variable button size: Button size is based on the graphic used for the button. Change the graphic, change the button. Most themes come with graphics to go with their theme, use those.

I'm having the same problem as lechinex, how do I export the menu items to other themes? Am I going to have to reconfigure it everytime I change themes??

Nice background
I like the idea of the blue circles for the side bars, however, they should match the background image's dark blue halo.
The start bar is boring compared to the background. I think the start button is a nice tie in to the background
